Start from one square image
Every size is made from the same source, so start with a square image at least 512 pixels wide with a simple shape: at 16 pixels, fine detail and small text disappear.
Transparency
The .ico and Android icons can be transparent. The Apple touch icon should not be, because iOS fills transparent areas with black; give it a background colour.
